Vote by Mail Application Form

• You’ll receive: the outer envelope, the inner envelope, and the ballot itself.

• You can find our Democratic ticket in Column B.

• Fill out the bubbles completely with blue or black ink. Don’t cross out or check-mark the bubbles.

• Insert your ballot into security envelope and seal it shut. Don't detach the flap.

• Sign the flap on the security envelope and print your name, address, and contact information.

• Insert security envelope into mailing envelope and seal it shut. Make sure the barcode is showing through.

• If you want to return the ballot by mail, affix a stamp and place it in the mail.

• Or, take your ballot to one of nearly 20 ballot dropboxes throughout the county. Nearly every town has one, usually at city hall.
